Your First Visit

Your first visit to Gary J. Wokuluk, D.D.S. establishes a vital foundation for our relationship with you. During this visit, Dr. Wokuluk will do an examination of your teeth, then x-rays will be taken (if applicable), and then there will be a consultation with Dr. Wokuluk where he will explain your diagnosis and treatment options. Occasionally, treatment can be performed the same day as the exam and consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on a separate day.

You can even save some time at your first visit by visiting our Patient Forms page in advance of your appointment. Download the forms and fill them out at home where your necessary information is handy and readily available. Then bring the filled out forms with you or you can fax them to us anytime before your appointment date.

Your Appointment

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Bring your insurance information and a list of your current medications if you haven't filled out your forms in advance. Please alert the office if you have a medical condition that may be of concern prior to any treatment (i.e,. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.).

IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the first / consultation visit.

X-Rays: If you have x-rays from a previous dentist or physician, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
